The service

Form For All lets you save your details once and use them to fill out participating clinics' forms via QR code, and lets clinics receive those submissions digitally. The service is provided as-is while in early access, without warranties of any kind.

Your responsibilities

You're responsible for the accuracy of the information you save and submit. A form submission goes to the clinic you submitted it to — check you're submitting to the right place. Clinics are responsible for handling received submissions in line with their own privacy obligations.

Acceptable use

Don't submit forms on someone else's behalf without their permission, and don't attempt to access data that isn't yours.
